Diabetes Programs

Living with Diabetes
This education program is open to all patients and their families with Type 2 Diabetes or Pre-Diabetes to participate in a group setting where they will learn more about their diagnosis and how to stay healthy and prevent the complications of diabetes. The goal of this program is to provide patients with the knowledge and skills to facilitate self-management of optimal blood glucose control. The common topics covered include pathophysiology, symptom management, blood sugar testing at home, oral medications and insulin, nutrition, physical activity, weight management, proper foot care and the stress/anxiety that can be associated with diabetes.
Nordic Pole Walking
This program invites patients with Type 2 Diabetes or Pre-Diabetes to join in a group exercise program that helps patients obtain the many health benefits of nordic pole walking as well as the support and accountability of walking with supervision from staff who are certified instructors. The program is run over several weeks and takes place outdoors as well as indoors during bad weather.
