What is a Family Health Team?
A Family Health Team is a team-based approach to health care that brings together family doctors and other health care professionals to provide patient’s with the best possible care.
Family Health Teams are able to meet patient’s needs by working together and combining expertise to offer care that is comprehensive, accessible, and timely.
Our Family Health Team
Comprising 14 physicians and 13 additional health professionals, our mission is a multi-disciplinary healthcare organization that provides Milton residents with integrated services to improve overall patient health & access to care.
